This academy demonstrates the different options for IOL fixation without capsular support. It also highlights the options of anterior and posterior optic capture and presents the plethora of indications for primary sulcus implantation in phakic and pseudophakic eyes for refractive purposes. The presenters will show surgical techniques, enlighten the rational behind each technique, give an overview of the results in literature and discuss the pros and cons of their described technique critically.

    IOLs are not implanted into the capsular bag in all our cases. There are several situations, when we implant an IOL into the sulcus, sometimes utilising anterior or posterior optic capture. Especially in children, we may implant the IOL into the bag, but utilise posterior optic capture to avoid after cataract. Even in phakic or pseudophakic eyes we sometimes implant IOLs for refractive reasons into the sulcus. In cases without sufficient capsular support, in aphakia and in cases with dislocated IOLs we need to find the best surgical solution for each patient.
