Proliferative vitreoretinopathy after retinal detachment surgery remains the main cause for recurrence. Certain therapeutic approaches have been proposed but there are no clear recommendations. Different treatment options will be discussed.
Then postoperative reattachment of the retina may be incomplete. The persistence of the subretinal fluid particularly in the macular area will limit the visual recovery. Wait and see or re-operate is a major question. Are there medical treatment options? Different approaches will be highlighted on the base of practical cases.
In other situations, despite a favourable anatomical outcome, an unexplained visual acuity loss is observed. How can we explain and prevent this disappointing result for both patients and surgeons? Are there any treatment options?    
In retinal vascular disease, despite panretinal laser coagulation and intravitreal injections, a very severe neovascularisation can challenge the functional and anatomical outcome of the eye. What treatment options do we have and in which order should they be used?
Finally, submacular bleeding either due to neovascular macular degeneration or to retinal macroaneurysm, the medical or surgical procedure remains to be defined according to the clinical presentation. Is less more? Should anticoagulants be discontinued or is a treatment for arterial hypertension to be considered?
