Smartphone ophthalmoscopy: present and future directions

Retinal imaging has considerably improved since the first photographic images of the ocular fundus were taken near the end of the 19th century. They are usually taken with bulky equipment requiring patients to be seated upright, which is often difficult for hospitalized or bedridden patients. To overcome these limitations, we took advantage of physicians’ pervasive adoption of smartphones, which are equipped nowadays with state-of-the-art cameras. We devel- oped a small optical device, which is attached magnetically to a smartphone, to conveniently examine and record videos or photographs of the retina.

Insufficient diabetic eye disease screening in low- and middle-income countries puts millions of patients with diabetes at risk of irreversible vision loss. Therefore, novel screening strategies including task-delegation and tele-medicine are urgently needed. Smartphone-based fundus imaging (SBFI) could potentially fill this gap, as it allows for low-cost mobile fundus examination using an adapter on a smartphone and delegation to paramedical staff. We evaluate the cost effectiveness of a telemedical smartphone-based diabetic eye disease screening program in India.

Age-related macular degeneration (AMD) is the leading cause of irreversible blindness in adults aged over 50 years. Disease Management represents a daily life challenge for patients and relatives. Adherence phenomenoms, patient information and quality assurance represent major topics. Real-life clinical outcomes of patients treated with IVOM therapy are often inferior to results from randomized clinical trials. Consequently AMD-Netz developed and provides the Makula App, whose outcome and perspectives are under evaluation.
